This policy explains how Olivia Ensor, trading as Olivia Ensor Advanced Sports Remedial Therapist ("we", "us", "our"), collects, uses, stores and protects your personal information, including sensitive health information, when you use this website or book an appointment. We handle your information in accordance with the Australian Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs).

When you complete a booking or enquiry form, we collect:

• Contact and identity details, such as your name, date of birth, profession, phone number and email address.
• Health information, such as the area of concern, pain level, treatment history and goals you tell us. Health information is "sensitive information" under the Privacy Act and is given a higher level of protection.
• Technical data, basic, non-identifying information about how the site is used (see "Cookies and analytics").

2. Why we collect it, and your consent

We collect this information so Olivia can assess your needs, contact you to arrange your appointment, prepare for your treatment and provide safe, appropriate care. By submitting a form and ticking the consent box, you consent to us collecting and handling this information, including your health information, for these purposes. Providing your information is voluntary, but if you choose not to, we may be unable to arrange or provide treatment.

3. How we store and protect it

Information you submit is transmitted over an encrypted (HTTPS) connection and stored using reputable service providers. We take reasonable steps to protect it from misuse, interference, loss and unauthorised access or disclosure, and we keep it only as long as needed for your care and to meet our legal and professional record-keeping obligations. When it is no longer required, we take reasonable steps to destroy or de-identify it.

4. Who we share it with

We do not sell your information. We share it only with service providers that help us run the practice, under confidentiality obligations, including:

• Our email provider, to send you a confirmation and to notify Olivia of your enquiry.
• Our website host and a bot-protection provider, to keep the site and its forms secure.
• Your private health fund, only where you ask us to help process a rebate.

We may also disclose information where required or authorised by law. Where a service provider stores data outside Australia, we take reasonable steps to ensure it is handled consistently with the APPs.

5. Cookies and analytics

This site uses privacy-friendly, aggregate analytics to understand general usage, such as which pages are visited. These analytics are cookieless and do not identify you personally.

6. Accessing or correcting your information

You can ask to see the personal information we hold about you, or ask us to correct it, at any time. Contact us using the details below and we will respond within a reasonable time. If we are unable to give you access, we will explain why.

If you have a concern about how we have handled your information, please contact us first so we can try to resolve it. If you are not satisfied, you can contact the Office of the Australian Information Commissioner (OAIC) at oaic.gov.au.
